Product Application&Function

Apixaban is the third new oral anticoagulant to be marketed after dabigatran and rivaroxaban. It has been approved in Europe to prevent venous thromboembolic events in patients undergoing elective hip or knee replacement surgery. Of the three oral anticoagulants approved in Europe, livaroxaban and Apoxaban demonstrated advantages in the RECORD and ADVANCE trials, respectively, over enoxabin, the current standard treatment for the prevention of venous thromboembolism after orthopedic surgery. When the results of the trials were viewed side by side, rivaroxaban appeared to be slightly more effective, but bleeding was worse than apixaban, experts said. Experts attributed these differences to the time of administration. In the RECORD trial, the drug was administered 6 to 8 hours after surgery, while in the ADVANCE trial, apixaban was administered 18 hours after surgery. The closer the time of administration of these drugs to surgery, the better the efficacy, but the higher the risk of bleeding risk. Bristmyers squibb/Pfizer touted the duration of apirxaban in ADVANCE, saying it is the only oral anticoagulant with a 12-24 hour window of initial treatment after surgery, which allows physicians to observe and stabilize patients before they begin treatment. However, rivaroxaban, a new once-a-day oral anticoagulant, has its advantages. Another anticoagulant, dabigatran, did not show comparable orthopedic results with the two anticoagulants. Dabigatran was shown to be as effective as the current standard of care in only two studies and was lagging behind in others. Dabigatran, however, is the first new anticoagulant to be approved by the U.S. Food and Drug Administration (FDA) for use in patients with nonvalvular atrial fibrillation (NVAF) in the United States and Canada.
